Spring Valley Health and Rehabilitation Center is a community based, non-profit, Medicare and Medicaid certified skilled nursing facility. We offer personalized nursing care, rehabilitative services, and supportive services on a beautiful campus which is professionally managed and set in a rural area that in itself promotes healing and recovery. We pride ourselves in offering the most flexible options for family, loved ones, or yourself. Our specialized services and person directed programs promote and assists in a person's recuperation from illness, injury, or surgery. Our friendly experienced staff of Professional Nurses, Certified Caregivers, Registered Therapists, all play a vital role in a Residents care planning team. It is the goal of our Rehabilitation Therapies to ensure that Residents reach their highest level of independence enabling them to return home after a period of rehabilitation.
Our facility also has the most current state of the art equipment available to focus on your well being and enrichment of daily life. Professional Nursing Staff consists of RN's, LPN's, and
Certified Nursing Assistants. Staff is
required to obtain continuing
education not only to re-enhance professional skills but to learn latest
developments and approaches. Our professional Nursing Staff is on duty
24 hours a day, 7 days
each week to monitor each Resident's medical needs.
Physical, Occupational, and Speech
Therapies are provided by
Registered Therapists who are a vital part of the
care planning
team. Rehabilitation Therapies help to ensure that Residents
reach their highest level of independence enabling them to return
home after a period of Rehabilitation.
Our Activities Department develops an activity program for
eachindividual based on each person's individual needs and preferences.
Activities include dances with live music, birthday parties, weekly
Bingo games and baking, movies, and book reviews. Community
groups visit and entertain Residents regularly. Weekly Religious
Services are conducted by local clergy persons. The Health Care
Center Van enables Residents to go on frequent outings including
fishing trips, shopping trips, sight seeing to areas of beautiful scenery,
and everyone's favorite going out to eat at area restaurants.
A large conference area is available for individual Resident Family Parties and get-togethers.
The Housekeeping and Maintenance Staff ensure that safety
and cleanliness
is maintained in a comfortable, warm, friendly home-like
environment.
An in-house Laundry Department provides personal laundry
service.
Local beauticians and barbers provide professional hair
care services in a
fully equipped Beauty and Barber Shop.
A specially adapted Medi-Van with a lowered floor and a ramp
system assists Residents with transportation to appointments such
as specialty clinics, eye exams, dental appointments, and other
non-emergency transfers to a hospital or clinic when the Resident or
their Family is unable to do so.
Good
nutritional food is an essential element in maintaining a healthy
body.
A consulting Registered Dietitian and a Certified Dietary
Manager assess and
manage each Resident's individual nutritional
program. Accommodations
are made for each individuals tastes and
needs. Various Physician
ordered "special diets" are prepared by
our Dietary Staff. The Spring
Valley Health Care Center prides itself
on its delicious home styled cooked
meals.
